To get a copy of the Address Request Form, you may:

visit the Department of Planning and Development at 102 West Main Street in the City Hall Annex in downtown Prattville. Address Request Forms are available at the front counter.

Once your completed form is received, a number will be assigned for the new address or addresses.*

Once the new address number is determined, Autauga County E911 will be notified. Autauga County E911 notifies all affiliated local government and utility agencies of the existence of the new address number(s).
